This is a patient diary written by Mark, 58, from Leeds. He spent £14 on paracetamol and saved over £20,000 by getting his All-on-4 done in Sofia. This is his account — edited for length, unedited for honesty.
The beginning: why I even started looking
My dentist had been warning me for three years. My upper teeth were deteriorating — three crowns had failed, two more were cracked, and the bone under one molar was too compromised to save. The plan: extract everything, wait for healing, then All-on-4 implants on the upper arch.
The quote: £22,000.
I earn a reasonable salary. But £22,000 for teeth — after tax — is roughly four months of take-home pay. My wife suggested I look abroad. I resisted, then I looked.
Month 1: Research
I spent two weekends reading everything I could find. Forums, Facebook groups ("Dental Tourism UK Experiences" has about 90,000 members and is genuinely useful), YouTube diaries, BalcanCare comparison articles.
Three destinations kept coming up for All-on-4: Bulgaria, Hungary, and Turkey. I narrowed it down to Bulgaria because:
- •EU member state, so EU medical device regulations apply
- •Sofia is a 3-hour direct flight from Leeds Bradford
- •Multiple people in the Facebook group had been to Sofia specifically and had positive experiences
I shortlisted three clinics, all of which I found via BalcanCare. I sent identical inquiry forms to all three, describing my situation and asking for a cost estimate.
The quotes
All three replied within 48 hours. One (the cheapest) gave me a quote with a long list of exclusions in small print. I asked for clarification — they never replied. Eliminated.
The other two:
Clinic A: €6,800 for upper arch All-on-4 (4 implants + full-arch bridge in zirconia). Consultation fee of €100 credited against treatment. Includes 2 years of follow-up.
Clinic B: €7,400 for the same. More comprehensive initial workup (3D CBCT scan included). English-speaking coordinator.
I chose Clinic B. The €600 difference is roughly one flight plus two nights accommodation. The coordinator at Clinic B answered every question I had (I sent about 14 emails before booking) with patience and detail. That mattered.
First trip: consultation and extractions
I flew to Sofia on a Thursday morning (Leeds Bradford → Sofia, 2h 55m, easyJet, £89 return). The clinic picked me up from the airport — they offer this for free, which I hadn't expected.
Day 1: Consultation The clinic is in a residential area, about 20 minutes from the centre. Modern, clean, smelled like a proper medical facility. I was taken through a full examination: 3D CBCT scan (shows bone density and structure), photos, and a 45-minute consultation with the lead implant surgeon, who trained in Vienna and spoke excellent English.
He agreed with my Leeds dentist's assessment: all upper teeth needed to go. He showed me the scan and explained exactly where the 4 implants would be placed. He was not dismissive of my questions. He did not oversell.
Day 2: Extractions Nine teeth removed under local anaesthetic with oral sedation. I won't pretend this was pleasant — it wasn't. But it was professional, and importantly, the post-op pain was managed well. I was given a temporary denture the same day (included in the price) so I wasn't toothless for the healing period.
I spent two more days in Sofia recovering at the apartment I'd booked (€65/night in Lozenets, walking distance to good restaurants where I could eat soup). I flew home on day 5, slightly swollen but fine.
The healing gap
All-on-4 requires bone healing before the final bridge is fitted. In my case, the surgeon wanted 4 months. Some people get immediate loading (final bridge within days) but given my bone loss, a staged approach was recommended.
During this period I wore the temporary acrylic denture. It was functional. I wouldn't call it comfortable. This is the reality of the process that not enough people discuss — there is a months-long gap where you have temporary teeth, and they feel temporary.
I had a video consultation with my coordinator at week 6 to share photos and check progress. Minor adjustment to the temporary denture was needed — I went back to Sofia for a day (flew out, adjusted, flew back same day). Cost: one day return flight.
Second trip: implants placed
Four months after extractions, I returned to Sofia for the implant placement surgery.
Day 1: Implant surgery Four titanium implants placed under local anaesthetic and IV sedation. I remember nothing. I woke up in the recovery room feeling groggy but not in pain. The procedure itself took about 2 hours.
The implants used were Nobel Biocare Active — the same brand used by top UK clinics. I asked specifically about this beforehand.
Day 2–3: Recovery Stayed at the same apartment. Swelling peaked on day 2. Not catastrophic — I looked like I'd been lightly punched. Ibuprofen and paracetamol managed the discomfort. I had no infection, no dry socket, no complications.
Day 4: Review scan 3D scan to confirm implant positions. Everything in exactly the right position. Surgeon seemed quietly pleased.
Flew home on day 5.
Third trip: the final bridge
Three months after implant placement (seven months from first visit), I returned for the final stage.
Day 1: Impressions and shade matching The in-house dental laboratory took digital impressions. We chose the shade together — they showed me options against my lower teeth. This took about 90 minutes.
Day 2: Try-in The bridge in its trial form was placed. We checked fit, bite, phonetics (saying "f" and "s" sounds, which reveal how the bridge contacts the lip). Minor adjustments to one tooth shape.
Day 3: Final fitting The finished zirconia bridge was screwed onto the implants. Torque-checked. Access holes sealed.
The coordinator handed me a mirror. I hadn't had a complete, good-looking upper set of teeth in probably eight years.
I cried a bit. I'm not embarrassed about that.
The final numbers
| Item | Cost | |---|---| | Full treatment (Clinic B, upper arch All-on-4, zirconia) | €7,400 | | Three return flights (Leeds–Sofia) | £267 | | Accommodation (6 nights total across 3 trips) | €390 | | Food, transport, incidentals | ~€250 | | Medication (antibiotics, pain relief) | ~€40 | | Total | ~€8,350 / ~£7,100 |
UK equivalent quote: £22,000
Saving: approximately £14,900
What I'd do differently
1. Book accommodation closer to the clinic. The clinic is in a residential neighbourhood. Staying near the centre added unnecessary taxi trips when I wasn't feeling well.
2. Allow more recovery days after extractions. I booked day 5 as my flight home. Day 4, I still felt rough. Day 6 would have been better.
3. Get everything in writing upfront. The clinic was honest throughout, but I learned to always get the full treatment plan with all costs itemised before paying any deposit. Surprises (however minor) are stressful when you're abroad.
4. Prepare your UK GP. My GP was initially a bit dismissive of the whole enterprise. I wish I'd had a more proactive conversation before travelling, particularly around prescribing antibiotics and managing the healing phase from their end.
Would I do it again?
Without hesitation.
The clinical standard at the Sofia clinic was indistinguishable from the best private dental care I've received in the UK. The materials, the technology, the surgeon's skill — all top-tier. What was different was the price, the wait time (booked within 2 weeks vs. 3-month waiting list in Leeds), and honestly, the personalised attention.
Seven months of my life, three trips to Sofia, and £7,100 later, I have a complete set of upper teeth that fit perfectly, look natural, and will likely last me the rest of my life.
That's not a bad deal.
